Obituary for ELOUISE Rosemary COOKS

Elouise Rosemary Cooks was born on November 28, 1938 to Hazel McClendon Baldwin and John Evans in Vienna, Georgia.

Elouise attended the Public Schools of Duval County. She was employed with Riteway Laundry Cleaners and retired after 45 years of dedicated service. After retirement Elouise became a loving foster parent. Elouise met and married Henry Hayes, Sr., from this union one child was born. She later married John Cooks.

Elouise was peacefully called to Heaven on January 29, 2017. She was preceded in death by her mother, Hazel McClendon-Baldwin; father, John Evans; husband, John Cooks; and sister, Thelma Owens.

Elouise leaves to cherish her memories: loving daughters, Betty Mitchell, Barbara Stevens, Priscilla Dumas, Debra Johnson, Brittany Cooks, Monica Cooks, Autrail Cooks, Brenda Webb (George), Corine Addison (Rev. Willie Addison), Elouise Cummings (Ike), and Helen Prince; sons, Henry Hayes, Jr. (Barbara), Randy Hayes (Anna), and Tom Monts (all of Jacksonville, Florida); sister, Sarah Lee; brothers, Roy McClendon (Betty), Coy McClendon (Maybell), and Willie C. McClendon (West Palm Beach); grandchildren, Rickey, Latoya, Collie Jr. (Shante), Georgeanna, George, Jessica, Erica, Evrin, Adrianna, Crystal, Derrick, and Laderricka; 24 great-grandchildren; godsons, Anthony McCoy and Michael McGriff; and a host of other loving relatives and friends.

The funeral service for ELOUISE Rosemary COOKS will be held 11AM, Saturday, February 4th, 2017 at the First Chronicles Baptist Church 2559 West 30th Street. The visitation will be 5 - 7PM , Friday, at the mortuary. Interment in Evergreen Cemetery. Please sign the family guest book and view the video tribute @ www.tswarden.com.
